Inspection forms and guides

These are the documents that we use in our inspection work. They include: detailed reading for inspectors; introductory reading for providers and users; forms for inspectors; forms for providers; and forms for others – for example, staff, pupils and parents and carers.

Registration forms and guides

These are the documents that we use in our registration work. They include: forms, detailed guides, leaflets and factsheets for prospective providers; forms, leaflets and factsheets for registered providers; and introductory reading for users of registered providers.

Other forms and guides

These documents include: forms and guides for complaints; the Compliance, investigation and enforcement handbook; incident notification forms.

Getting a form or guide

Our forms and guides are available as a free download; most are in Word and/or PDF format. In addition to this:

  • an inspector may provide copies of some forms and guides to a provider when carrying out an inspection; this may include those for others, such as staff and parents and carers.
  • free printed versions of some forms and guides for prospective providers when applying to register are available. For further information, go to the appropriate provider completing your application page:
